Health Perception and Management is the first of Gordon's Functional Health Patterns, which is a method invented by Marjory Gordon to be used in the nursing process to provide a comprehensive nursing assessment of the patient. Understanding Health Perception & Helath Management for Nurse is vitaly important to providing care to the patient.
Health Perception – Health Management; Describes client's perceived pattern of;
- What is health ? ,
- What is wellbeing ? , and
- How health is managed ?
Data collection is centered on the level of health and well-being one’s perceived, and what practices he/she choose to maintain health. Habits, injurious to health are also evaluated, including smoking, food, life style and alcohol or drug use. Any problems related to safety and health management may be identified as well as needs for modifications in the home or needs for continued care in the home.
The following questions pertain to those asked by the nurse to provide an overview of the individual's health status and health practices that are used to reach the current level of health or wellness.
Taking History (subjective data):
1. Client’s general health?
2. Any colds in past year?
3. If appropriate: any absences from work/school?
4. Most important things you do to keep healthy?
5. Use of cigarettes, alcohol, drugs?
6. Perform self-exams, i.e. Breast/testicular self-examination?
7. Accidents at home, work, school, driving?
8. In past, has it been easy to find ways to carry out doctor’s or nurse’s suggestions?
9. (If appropriate) What do you think caused current illness?
10. What actions have you taken since symptoms started?
11. Have your actions helped?
12. (If appropriate) What things are most important to your health?
13. How can we be most helpful? How often do you exercise?
For Objective data, Nurse can observe and record any symptoms, habits and attributes which are not limited to daily routine but also attractions towards the certain norms about health.
These assessments of Health Perception and Management can enable a nurse to understand the Client / patient health agenda and with the appropriate changes nurse can lead Client / patient towards healthy life
